Veronica Filardi, Lighthouse Administrator

We are pleased to announce that Veronica Filardi, BSN, has been appointed as the new Lighthouse Home Health Care Administrator, effective January 29, 2018. Mrs. Filardi received her Bachelor of Science in Nursing from Fairfield University. She has experience in critical care, cardiology, hospice care, medical/surgery, telemetry, and over 20 years’ experience in home health care. She has demonstrated her leadership with providing clinical supervision of nurses, nurses’ aides, therapists and social workers.

Mrs. Filardi has coordinated home care services with discharge planners, physician offices, hospitals and skilled nursing facilities and has been responsible for the growth of past agencies while improving their Medicare 5 Star Rating. She has experience in maintaining a quality improvement program to comply with state and federal laws with over 90% compliance. Mrs. Filardi has extensive experience with OASIS review and coding, as well as chart review and auditing for positive outcomes and to prevent rehospitalizations.

Mary Koziel, Clinical Manager

We are pleased to announce that Mary Koziel, BSN, has been appointed Clinical Manager at Lighthouse Home Health Care (Stratford) effective January 22, 2018. Mrs. Koziel has over 35 years as a Registered Nurse, 25 years of which she served as a Supervisor.

Mrs. Koziel has various experiences in the healthcare field including hospitals, schools, skilled nursing facilities, and home health agencies. She has experience in OASIS review, insurance verification, staff education, Infection Control, and Quality Assurance. She also has experience in managing a large staff with a high-volume census.

LHHC Receives 4.5 Star Rating In December

The Quality of Patient Care Star Rating is based on OASIS assessments and Medicare claims data. These ratings were first published on HHC in July, 2015. Patient Survey Star Ratings are based on the patient experience of care measures.

Frank Raccio, Executive Director

Mr. Frank Raccio has recently joined Lighthouse Home Health Care and Companion Care as the Executive Director.

He has a Bachelor’s degree in Athletic Training, a Master’s degree in Exercise Science, an Associate’s degree as a Physical Therapist Assistant and is a Licensed Nursing Home Administrator.
Mr. Raccio has been with Ryders Health Management for 8 years, beginning his employment with Ryders as the Director of Rehabilitation at Bel-Air Manor. While at Bel-Air Manor, he developed and expanded both the inpatient and outpatient rehab programs and staff.

He then transferred to Lord Chamberlain where he converted the Rehab department from contract therapy to an in-house model and restructured the Rehab department to provide optimal rehab services to assist the recovery of short term and long term patients.
While at Lord Chamberlain, Mr. Raccio created and designed a stand alone outpatient business for Ryders Health Management, called Ryders Rehabilitation. Unlike the Skilled Nursing Facility outpatient model, Ryders Rehab was created with a community-based referral source in mind and with the goal to expand into Assisted Living communities and complete the continuum of care that Ryders Health Management offers.

In his newest role, Mr. Raccio will continue to oversee Ryders Rehabilitation as well as activity at both Lighthouse Home Health Care and Lighthouse Companion Care.

“Two-thirds of Connecticut’s 99 licensed home health care agencies provide average or above-average care, while 19 were rated below average, according to new Medicare five-star rating data.

Just one agency, McLean Home Care & Hospice in Simsbury, received the highest rating of five stars; three agencies, including the Lighthouse Home Health Care in Old Saybrook, received 4.5 stars;”
